Continuing Care

Continuing CareThe completion of a SageWalk program is only the beginning. Our students, most of who tend to be bright, verbal and manipulative, learn many things while in the SageWalk program. They identify patterns that caused difficulty at home and learn new forms of healthy expression. While the old patterns can survive and undermine the student’s successful transition back home, the new pathways are powerful yet fragile until successfully proven and accepted. For these reasons students and parents need, and have asked for, continuing support to facilitate effective change and transition.

The SageWalk Continuing Care Program offers a multifaceted approach to supporting families following graduation.

  • All parents are provided with written materials designed to help them learn and practice new skills at home, while their children are growing and learning in the wilderness environment. The SageWalk Therapist working with the family will guide parents through this learning process as parents prepare for graduation.
  • The Family Graduation Workshop provides opportunities to further develop and practice these new family skills while celebrating the successful completion of the program.
  • A Continuing Care Mentor will contact the family regularly after graduation to provide support in conjunction with local professionals working with each family. These calls are scheduled individually with parents and students bi-weekly. Mentors will also provide support in strategizing how to successfully implement and maintain SageWalk’s therapeutic recommendations for each graduate.
  • Weekly Parent Teleconference Calls: A trained staff member facilitates a group conference call and discussion on a different topic each week. The topics are based on parent recommendations and provide support and understanding as families meet the challenges of life after SageWalk.
  • Crisis Support – This is for parents or students who encounter a difficulty at home and have tried to solve the problem on their own but still feel they need some help. They can contact SageWalk and one of our Continuing Care Mentors will provide that support.
Wilderness School Continuing CareThese components are offered to all SageWalk Transition Program graduates for the first 3 months following graduation. If a family feels continuing with this support after 3 months would be valuable for them, the Mentor Calls, Teleconference Calls and Crisis Support can be purchased for an additional fee.
